Registration
and fee information
Day Trips
Day
trips have become a popular option for some of the weekends.
Depending on the group, day trip activities may be scheduled without
additional cost. If additional money is needed, you will be notified
in advance. Groups have gone out for pizza, to a college football
game, bowling, out to dinner, and to an ice show. Other options may
include: A Saturday matinee, a visit to a museum, roller skating, ice
skating and more...
Fees
-
A $150
registration fee is requested and will be applied to the total cost of
the weekend.

The cost
for the weekend is based on the level of supervision and care needed,
living situation, and eligibility for county contract respite funding
for the individual. Please contact our office with any
questions.

All fees
paid in advance will be refunded in full if notice is received in our
office 30 days prior to the applicant's session. If less than 30
days notice is received, all but the registration fee will be refunded.

Fees can
be pro-rated if the person will not be attending the entire
weekend. Please indicate the length of stay on the application
form or by contacting the office directly.

Note
to parents: If your child is eligible to receive county funds for
respite care or foster care, please contact your social service
agency, as these funds may be applied toward the respite weekend fees.
How
to Register

Registrations
will be accepted on a first-come, first-served basis.

To
ensure a quality experience, only a limited number of people who have
behavioral considerations, or require a great deal of personal or
medical care will be accepted.

If your
application is accepted, a confirmation packet will be sent to
you. Enclosed in the packet will be a letter confirming respite
weekends and dates; information concerning check-in and check-out
procedures and times; clothing lists; map with directions; a physical
examination form; and a physician's standing order form.

A
physical examination form is required for all respite participants no
more than 24 months prior to attendance.
